During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 03/31/2026 and completed on 04/08/2026. The Department issued a Special Order on 02/26/2026 to complete training and contact parents as a result of high-risk violations. The on-site inspection began at 10:25 a.m. and ended at 12:05 p.m. The inspector reviewed compliance in the areas listed above. There were 25 children present and 6 staff. The inspector reviewed 5 children?s records on-site and 5 staff records electronically on 04/08/2026. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present.

Child 5, with a diagnosed food allergy, was present and no list was available.
Plan of Correction: Staff will create a list available for staff.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child 5 (enrolled for six months) has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: Provider will obtain from parent.
Standard 8VAC20-821-270-6-f
A copy of any special order issued by the department shall be posted in a prominent
place at each public entrance of the licensed premises to advise consumers of serious or
persistent violations.

The special order issued by the Department on 02/26/2026 was not posted.
