Inspector Notes

An unannounced, on-site focused inspection was initiated on 10/4/2024 and completed on 10/4/2024 . The on-site inspection began at 11:01am and ended at 12:04pm. The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was 14. The inspector reviewed five children?s records and one caregiver and one household members records on-site on 10/4/2024. This inspection included document review , tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/17/2024. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION

Background checks are required every 5 years for any persons involved in the day-to-day operations of the family day home and supervision of children.

The provider?s central registry search was last completed six years ago and is now 1 year overdue.

REPEAT VIOLATION

Equipment used by children were maintained in accordance with the manufacturer?s instructions and that equipment shall be free from hazards such as lead paint, sharp edges or points, loose parts, and rust.

Splitting and splintering wood was observed on several areas on the wooden treehouse in the backyard and a picnic table in the play area in the backyard had a cracked tabletop.
