Inspector Notes
An unannounced Monitoring inspection was conducted today. A total of 10 children (28pts) with 2 staff were present. The children were observed playing in the programming space with toys and equipment within reach to enable independent selection. One child was observing sleeping in the crib room. Observation of the crib room, both nap rooms, program rooms, diapering area, outside play space and kitchen was made. The older children were observed transitioning to a learning activity with the provider and the younger children were supervised by the assistant. Hand washing procedures were observed prior to sitting down for lunch. One medication need currently. A total of 5 children's records and all staff records (2) were reviewed. Previous violations from the last inspection were reviewed and corrective actions were verified for all but one repeated issue. This inspection began at approximately 10:30am and ended at 12pm. Child #1 and #3's immunizations were not signed by a physician or a physician's designee.
Evidence:
Blind cords located in the name room were within reach of children in care.
Evidence:
The gate located at the top of the steps leading to the side yard did not latch to prevent a child from falling down the steps.
Evidence:
A written record for each child each child in attendance was not maintained.
Evidence:
Two topical ointments were found that were not labeled with a child's name.
Evidence:
Child #2 began care on 08/22/2022. Immunizations were signed and dated by the physician on 08/29/2022.
