Inspector Notes

An unannounced Monitoring inspection was conducted today. A total of 7 children(22pts) with the Provider and 2 assistants were onsite. Some of the children were playing in one room and a smaller group were seated in the smaller program space completing a table top activity. Handwashing procedures and diapering procedures were observed during the inspection. The children transitioned outside to the side yard to play with 2 staff. The provider remained inside to bottle feed one infant. The side yard where the children play now has artificial grass to allow the children to play more frequently after inclement weather. A suggestion was made to add some additional topsoil to balance the grading underneath of the artificial grass. Staff were observed cleaning up from morning snack and proper cleaning and sanitizing of the table tops was observed. Observation of the bathroom, crib room and the two nap rooms was made. A total of 5 children's records and the provider's and 2 staff records were reviewed and found complete. This inspection occurred between 10am and 11:50am. Thank you for your cooperation today. Based on review of staff records, one subsequent TB screening was not obtained.
Evidence:
Staff #2's last TB screening was completed in September 2021. A subsequent TB screening is required every 2 years.
Plan of Correction: The staff will have a TB screening completed as soon as possible.
Standard 8VAC20-800-750-D
Based on a review of topical ointments, one ointment had expired.
Evidence:
A topical ointment for Child # 1 had expired in 2020.
Plan of Correction: This ointment will be disposed off and the parent will be asked to bring a new one in.
