A subsidy health and safety (SHSI) inspection was completed on 8/23/2023. A Voluntary Registered Monitoring inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Based on a review of records and interview, the caregiver did not undergo a criminal background check within 5 years of the most recent documented background check results.
Evidence:
The most recently dated criminal record check for caregiver #1 was dated 5/16/2018 which is more than 5 years ago.
The provider confirmed that she had not obtained an updated Criminal Record Search since 5/16/2018.

Based on a review of records and interview, the provider did not annually attend at least 16 hours of training, to include the department's health and safety update course.

The provider was unable to locate documentation of the department's health and safety update course. The original training was present in the record and was dated 1/21/2017.
