Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 8/28/2024 from 12:15pm to 3:30pm. At the time of entrance, there were no children in care with the vendor. The sample size consisted of 1 child record and 1 provider record.

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Standard 8VAC20-790-190-B
Based on a review of records and interview, the vendor did not ensure that the child's information included all of the required documentation.
Evidence:
One enrolled child's record was reviewed.
The provider confirmed that the record did not contain proof of the child's identity and she did not have this documentation available for review.
The provider confirmed that the record did not contain an immunization record which is required before the first day of attendance and she did not have this documentation available for review.
Plan of Correction: The vendor will request a copy of the child' #1s proof of identity and physical results from the parent and place it in the child's record. This child is a family member and the provider is certain of his identity and age.
Standard 8VAC20-790-210-B
Based on a review of records and interview, the vendor did not ensure that caregivers undergo TB screenings at least every two years from the date of the initial screening or more frequently if recommended by a physician.
Evidence:
One caregiver record was reviewed.
The record for caregiver #1 contained the most recent TB screening which was dated 8/18/2022, more than 2 years ago.
Plan of Correction: The vendor had a Tuberculosis Screening completed on 9/10/2024 and a copy was sent to the inspector.
Standard 8VAC20-790-250-F
Based on a review of records and interview, the vendor did not ensure that caregivers who work with children will, in addition to required preservice and orientation training, annually attend at least 16 hours of training, to include the department's health and safety update course.
Evidence:
The vendor confirmed that she did not have documentation of any training completed over the last 12 month period and had not completed the department's annual health and safety update course in the last 12 month period.
Plan of Correction: The vendor states that she will complete the department's annual health and safety update course in the next 30 days and maintain the certificate in her record. A minimum of 16 hours total training will be completed within 12 months.
Standard 8VAC20-790-430-B
Based on a review of records and interview, the vendor did not maintain a record of the dates of practice drills for one year.
Evidence:
The provider confirmed that she did not have documentation of the practice drills for the previous 12 months period. The provider stated that evacuation drills, shelter-in-place drills and a lockdown drill had been practiced, but she was unable to show producedocumentation of them.
Plan of Correction: The vendor states that she will begin documenting the monthly evacuation drills as well as shelter in place and lock down drills beginning in in September and will ensure that they are documented in a place where they are easily accessed for review..
