Inspector Notes
A subsidy health and safety (SHSI) inspection was completed on 8/28/2024 from 12:15pm to 3:30pm. At the time of entrance, there were no children in care with the vendor. The sample size consisted of 1 child record and 1 provider record.
A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

The provider confirmed that the record did not contain proof of the child's identity and she did not have this documentation available for review.
The provider confirmed that the record did not contain an immunization record which is required before the first day of attendance and she did not have this documentation available for review.
Evidence:
One caregiver record was reviewed.
The record for caregiver #1 contained the most recent TB screening which was dated 8/18/2022, more than 2 years ago.
Evidence:
The vendor confirmed that she did not have documentation of any training completed over the last 12 month period and had not completed the department's annual health and safety update course in the last 12 month period.
Evidence:
The provider confirmed that she did not have documentation of the practice drills for the previous 12 months period. The provider stated that evacuation drills, shelter-in-place drills and a lockdown drill had been practiced, but she was unable to show producedocumentation of them.
