Inspector Notes
An unannounced renewal inspection was conducted June 4, 2024 between 9:40 am and 11:00pm. There are no children presently enrolled in the program. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, evacuation drill logs, first aid supplies, attendance, household members and caregiver records, and emergency procedures. No medication on sight at the time of the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 1. Household Member #2 did not have a current TB screening on file. The last documentation on file was dated 12/27/2020 (Expired 12/27/2022).
2. The provider acknowledged the file did not have a current TB screening results.
