During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site renewal inspection was initiated and completed on July 2, 2025. The on-site inspection began at 2:20pm and ended at 3:40pm. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was seven. The inspector reviewed two children?s records and five caregiver/household members records on-site. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Pressure mounted gates shall not be used as protective barriers at stair openings.

A pressure mounted gate was being used as a protective barrier at the stair opening.
Plan of Correction: I will ensure updated immunizations are every six months for children two years and under.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. Child #1, 16 months old, last documented immunization was over eight months ago.
Plan of Correction: I will have the gate bolted to the wall by 07/07/25.
