Inspector Notes

An in person monitoring inspection was conducted today. There were 4 children (12 points) with one caregiver present. 4 child records and 5 household member records were reviewed. See Violation Notice for non-compliances.
Bathroom supplies, fridge temperature, emergency evacuation drills and posted documents were all in compliance. The inspection report will be typed off site and emailed to the provider. Based on a review of records, household member records do not contain documentation of repeated background checks within the 5 year allowable time frame.
Evidence;
Sworn Disclosure Statement (SDS) and Central Registry Searches (CPS) were not repeated within 5 years for household members (HHM).

Evidence:
2 of 4 records reviewed did not contain the correct address for the relocation destination in the event of an emergency, disclosed in the Emergency Preparedness Plan.
The address written on the Provisions of the Emergency Preparedness Plan for the destination of the relocation is not the correct address. (Children #1, 2) (model form provided)
