Inspector Notes
An unannounced monitoring inspection was conducted today.
There were 7 children present (20points) with 1 caregiver at LI arrival.
Children were observed finishing lunch, washing hands, playing with a variety of age appropriate toys.
Emergency evacuation drills, fridge/freezer temperatures, first aid/CPR certifications, first aid kit supplies, emergency supplies, CO2 monitor, and updated emergency evacuation plan were all in compliance today.
7 child and 2 caregiver records were reviewed.

5Evidence:
The TB result on file for Caregiver B is dated10/19/21. (expired)
Evidence:
2 of 6 crib mattresses were not covered with a waterproof material.
Mattress #1 (blue striped mattress) was covered with a cloth surface.
Mattress #2 had a large rip in the plastic covering, making it no longer waterproof or able to be cleaned and sanitized.
Based on observation, the provider did not ensure that a caregiver did not exceed 16 points to determine when an additional caregiver is needed.
Evidence:
At LI arrival (12:00 PM) the provider was alone with 20 points. A second caregiver arrived in the daycare area in approximately 5 minutes.
Based on a review of records, one child record was not maintained as up-to date.
Evidence:
There was no annual review and update documented for 1 of 7 records reviewed.
Child #4's record was dated 10/11/22.
Evidence:
A soiled diaper/pull-up was observed on the floor of the bathroom, next to the diaper disposal container.
