Inspector Notes

An unannounced monitoring inspection was conducted today.
There were 6 children present (15 points) with one caregiver. Children were observed during diapering, drinking milk, playing with toys and looking at books.
5 child, I caregiver and 1 household member's records were reviewed.

Repeat Violation
Based on a review of records, the provider did not obtain for each household member a current Report of Tuberculosis Screening form every two years.
Evidence:
Household member #1's TB test result was dated 10/12/21. (expired).

Standard 8VAC20-800-690-D
Based on observation, the diapering surface was not non-absorbent, cleaned and sanitized after diapering a child, and was a surface used for children's activities.
Evidence:
A child's diaper was changed on a paper towel, on the floor of the day care area. The towel was disposed of; however, the area could not be cleaned and sanitized, as it was not a waterproof surface.

Based on observation and provider statement, the records of emergency evacuation and shelter-in-place drills was not maintained.
