Inspector Notes
An unannounced monitoring inspection was conducted on 2/27/2024 from 4:30pm to 5:30pm. At the time of the inspection there were 20 children in care and 3 staff members. Children were observed participating in various activities including group choice games and homework. Records were reviewed for 5 children. Staff records were reviewed at the main office location on 2/28/2024. Evacuation drills, emergency supplies and other required records and postings were also reviewed. There was no medication on site during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
Violations
3Based on observation and interviews, it was determined the facility did not ensure the required nonmedical emergency supplies were available on each floor of the building.
Evidence:
1. The battery-operated radio was missing batteries and did not work.
2.Staff #1 confirmed the radio was not working.
Based on record review and interviews, it was determined the center did not maintain and keep at the center a separate record for each child enrolled with all required information.
Evidence:
1.There was no record available at the center for child #2.

Evidence:
1.During the inspection, staff were unable to provided documentation of emergency contacts for staff members.
