Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff #1, who did not meet lead teacher qualifications, was alone with a group of school age children for approximately 15 minutes.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for more than 3 months, was missing the following required components: the name, address and phone number of two emergency contacts, previous daycare and schools attended by child, and the required signed written agreements between the parent and the center.
