Inspector Notes
An unannounced monitoring inspection was conducted at 12:10pm. Observed 5 children totaling 19 points with 2 caregivers. Ratios and supervision were in compliance. Children were finishing lunch, washing hands and playing with toys during the inspection. Lunch served included chicken, mixed vegetables, bread, banana and milk. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Evidence:
1. Child #3-1 of 2 emergency contacts (name, address and phone) was left blank and the medical insurance number was left blank.
2. Child #4-The medical insurance number was left blank and the emergency medical authorization was not signed by the parent.
