An unannounced, on-site monitoring inspection was initiated on 8/27/2025 and completed on 8/27/2025, as a part of the licensure period. The on-site inspection began at 2:26PM and ended at 3:39PM. The inspector reviewed compliance in the areas listed above. There were 37 children present and 3 staff. The inspector reviewed 5 children?s records on site on 8/27/2025 and 3 staff records electronically on 9/9/2025.
This inspection included - document review, tour of the facility, and observations.

Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #2 was not signed.

Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The screening for Staff #2 was not signed by a physician.
