Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on July 27, 2022 and concluded on July 28, 2022. There were 55 children present, ranging in ages from infants to age 12, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: The following staff did not obtain a fingerprint as required. 1) Staff #1?s first date of employment was June 21, 2021 and there was not fingerprint check available for review. Staff #1 was observed working during the inspection.
2) Staff #3?s first date of employment was June 20, 2022 and the results of the fingerprint were dated June 27, 2022.
Evidence: Two out of five records reviewed did not have documentation of TB screening/ test available for review for the following staff that was conducted in the required timeframe.
1) Staff #2?s TB screening was dated March 2, 2022 and the first date of employment was February 25, 2022.
2) Staff #3`s TB screening was dated July 4, 2022 and the first date of employment was June 20, 2022.
Evidence: A television was observed sitting on a bookshelf and the book shelf was approximately 28 inches high. There was black plastic tape connecting the base of the television and the book shelf, however the television still moved easily when touched and is not secured to the book shelf and poses a safety risk to the children in care. This was observed in the two classrooms for ages 3 to 5 and in the two classrooms for ages 2 to 3.
Based on observation and staff interviews, the licensee did not ensure staff shall wash their hands with soap and running water before and after a diaper change.
Evidence:
Staff #2 was observed changing the diaper for child #6 and after their diaper was changed staff #2 took off their gloves and then used a baby wipe and wiped off the nose, mouth and face of child #6.
Staff #2 did not wash their hands as required.
