Inspector Notes

A monitoring inspection was initiated and concluded on May 10, 2023. There were 53 children present, ranging in ages from 5 months to age 5, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) was request by the 30th day of employment
Evidence: Staff #5?s record did not include an out of state CRC for Texas where they had lived within the last five years from hire date, 4-10-2023. Center Director confirmed the missing information.
Plan of Correction: The out-of -state background check has been processed.
Standard 8VAC20-780-40-M
Based on observation and staff interview, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence: Staff #4 confirmed there was not a list in the infant room documenting that child #2 had a food preference/ sensitivity.
Plan of Correction: The allergy list has been updated.
Standard 8VAC20-780-570-A
Based on observation and staff interviews, the licensee did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.
Evidence Child #3. age 5 months was observed in the an infant seat and the strap was not used.
Plan of Correction: The staff has been retrained.
