Inspector Notes
An unannounced monitoring inspection was conducted in the center on August 15, 2023 from 12:30 pm to 3:00 pm and concluded on August 22, 2023. At the time of entrance, 60 children were in care with 8 staff members present. The sample size consisted of 5 children?s records and 4 staff records. Children were observed preparing for and during rest period.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
5Evidence: The record for Child #1 did not have an immunization record and first date of attendance was April 24, 2023
Evidence: Center Director and Infant teacher, staff #5 confirmed on August 15, 2023, 4 infants played in a wading pool and only staff, Staff #5, was supervising the 4 infants. The requirement of a minimum of two staff was not met.
Evidence: Center Director and Infant teacher, staff #5 confirmed on August 15, 2023, 4 infants played in a wading pool and they are not toilet trained and were wearing swim diapers.
Evidence: Center Director confirmed medications, for Child #2 (medication albuterol) and Child #6 (medication, albuterol) were not labeled with the child's name, the dosage amount, and the time or times to be given.
Evidence: There were 5 separate groups of children observed and the following groups did not have attendance sheets indicating the children?s arrival and departure.
1.Staff #1 confirmed they did not have an attendance sheet for the 8 out of 16 children they were supervising during naptime in the two-year-old room.
2. In the 3?4-year-old classroom at approximately 12:40 pm, 10 children were observed in care and 11 children were listed on the attendance sheet. Staff #7 confirmed Child #7 had left the center at approximately 10:20 am, and the departure time for Child #7 was not documented and the attendance list was not updated.
