Inspector Notes

An unannounced monitoring inspection was conducted in the center on August 15, 2023 from 12:30 pm to 3:00 pm and concluded on August 22, 2023. At the time of entrance, 60 children were in care with 8 staff members present. The sample size consisted of 5 children?s records and 4 staff records. Children were observed preparing for and during rest period.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
Based on children?s record review and staff interviews, the licensee did not ensure to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: The record for Child #1 did not have an immunization record and first date of attendance was April 24, 2023
Plan of Correction: Immunization record has been received.
Standard 8VAC20-780-460-A
Based on staff interviews, the licensee did not ensure there were at least two staff members supervising the swimming/wading activity.

Evidence: Center Director and Infant teacher, staff #5 confirmed on August 15, 2023, 4 infants played in a wading pool and only staff, Staff #5, was supervising the 4 infants. The requirement of a minimum of two staff was not met.

Standard 8VAC20-780-470-E
Based on staff interviews, the licensee did not ensure that children who are not toilet trained may not use portable wading pools.

Evidence: Center Director and Infant teacher, staff #5 confirmed on August 15, 2023, 4 infants played in a wading pool and they are not toilet trained and were wearing swim diapers.

Standard 8VAC20-780-510-G
Based on observations and staff interviews, the licensee did not ensure medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Center Director confirmed medications, for Child #2 (medication albuterol) and Child #6 (medication, albuterol) were not labeled with the child's name, the dosage amount, and the time or times to be given.
Plan of Correction: Medication has been updated.
Standard 8VAC20-780-80-A
Based on documentation review and staff interviews, the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: There were 5 separate groups of children observed and the following groups did not have attendance sheets indicating the children?s arrival and departure.
1.Staff #1 confirmed they did not have an attendance sheet for the 8 out of 16 children they were supervising during naptime in the two-year-old room.
2. In the 3?4-year-old classroom at approximately 12:40 pm, 10 children were observed in care and 11 children were listed on the attendance sheet. Staff #7 confirmed Child #7 had left the center at approximately 10:20 am, and the departure time for Child #7 was not documented and the attendance list was not updated.
Plan of Correction: Staff will ensure when moving classrooms for nap that the sign timesheets will also be moved.
