Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 08/02/2024 and completed on 08/02/2024. The on-site inspection began at 12:25pm and ended at 2:34pm. The inspector reviewed compliance in the areas listed above. There were 68 children present and 13 staff. The inspector reviewed 6 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews and, observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 14,2024. A POC submitted after this date will not appear on the public website.

Staff #2 most recent sworn statement was 12/04/2018.
The sworn statement was more than five years old.
Based on observation and staff interviews, the
licensee did not ensure that children under 10
years of age always shall be within actual sight
and sound supervision of staff.
Evidence: Child #1 (age 3) was observed alone in the hallway at approximately 12:31 PM. LI
saw the child alone and told staff. Staff returned child#1 to class.
