An unannounced, non-mandated inspection was conducted on 2/15/2024 from 10:00am to 10:30am. The inspection was in response to a subsidy complaint received on 2/14/2024. The investigation was focused on compliance with the vendor agreement (8VAC20-790-520.A). Information gathered during the inspection determined areas of non-compliance with applicable standards or law. Violations were documented on the Violation Notice issued to the program. Based on an interview, the vendor was not in compliance with the standards in this part, the terms of the vendor agreement, and all relevant federal, state, or local laws and regulations. Evidence: The vendor agreement does not allow for vendors to retain subsidy cards or subsidy numbers for children enrolled under the subsidy program. Staff A stated a card was retained for Child A. Staff A stated the subsidy account numbers were retained at the facility for Child B, Child C, Child D and Child E.
Plan of Correction: We have contacted all of the subsidy families. We are aware of the policy now and will follow it.
