The center is required to have employees undergo a background check every five years.

The most recent fingerprint-based background check for staff #3 was overdue by two years and nine months.

A search of the sex offender registry and child abuse and neglect registry from any state in which the individual has resided in the preceding five years must be conducted prior to employment.

Staff #1 and Staff #4 resided in one state outside of Virginia in the preceding five years. The center did not conduct a search of the sex offender registry for that state prior to employment.

At least every two years from the date of the first initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.

Staff #1 TB screening is six months past due.

Staff #2 TB screening is three months past due.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Staff #2 and Staff #4 did not have documentation of completing the required 16 hours of annual training.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain: Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Child #3 emergency contacts were missing addresses.

Child #7 did not have a second emergency contact on file.

The following staff records shall be kept for each staff person: Documentation that two or more references as to character and reputation as well as competency were checked
before employment.

Staff #1 only have one reference documented.

Staff #2 and Staff #4 did not have any documentation of references being completed.

Current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration.

Staff #1 CPR certification expired four months ago.

Current certification in first aid appropriate to the ages of children in care.

Staff #1 first aid certification expired four months ago.

Staff who work directly with children shall, in addition to preservice and orientation training, annually attend at least 16 hours of training, to include the department's health and safety update course.

Staff #2 annual health and safety update training is two years past due.
