Inspector Notes

An unannounced inspection was initiated on 9/10/2024 and completed on 9/19/2024 in response to a complaint received by the licensing office on 9/9/2024 relating to allegations of inadequate supervision and staff to child ratios. The inspector reviewed 4 staff records on-site on 9/19/2024. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable code sections and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/22/2024. A POC submitted after this date will not appear on the public website.

The child day center is required to maintain a ratio of one staff member to four children from ages zero to 16 months.

Staff #1 was the only staff supervising nine children under 16 months of age in room #2 and room #4 with an adjoining door. Four infants were in room #1 without staff supervision. Two infants were in room #3 without staff supervision. Staff #6 was observed supervising 6 children (youngest child 15 months).
Plan of Correction: If put in this position again, parents will remain in parking lot with children until staff arrives.

The child day center is required to maintain a ratio of one staff member to five children from ages 16 months to 24 months.

Staff #7 was observed supervising 6 children (youngest child 23 months).
Plan of Correction: Wait until the actual day of the birthday for child to join the 2 yr old class which makes ratio 8:1.
