Inspector Notes

A renewal inspection was conducted on 9/26/2023 with the center director. There were 43 children present, ranging in ages from 5 years to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of inspection reports, the facility failed to ensure that an annual fire inspection was conducted by the appropriate fire official having jurisdiction. Evidence: The last available fire inspection on file was dated 4/29/2022. Staff checked with the school office and an updated fire inspection could not be located.

Based on review of 5 children's files, the facility failed to have a written care plan for each child
with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child 2 has diagnosed food allergies, but did not have a written allergy action plan on file.
