Inspector Notes

A subsidy health and safety inspection was conducted on 2/14/2023 between 11:30am and 12:15pm. There were three children receiving care by the vendor. The physical plant, polices and procedures, emergency supplies and procedures, emergency drills, 3 children's records, the vendor/provider's record, and 3 household member's records were reviewed. An exit interview was conducted with the vendor. Areas of non-compliance can be found on the violation notice. Based on record review, children's records did not contain all required information.
Evidence:
1. Child 1's record did not contain: the date the child entered care; one of the listed parent's work address; an address for the listed emergency contact; a signed emergency medical authorization.
2. Child 2's record did not contain: the date the child entered care; the name of an emergency contact person.
3. Child 3's record did not contain: the full name address, and phone number for one of the child's parents or the addresses for either parent's place of employment.
