A subsidy health and safety inspection (SHSI) was initiated on February 26, 2025 and completed on February 26., 2025. The inspection started at 9:20am and concluded at 10:00am. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 1 child?s record and 4 caregiver/household member(s) records [on-site].

Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.
