Inspector Notes

An unannounced monitoring inspection was conducted on 11/28/2023 from 9:24 am to 11:20 am.

There were 28 children supervised by 8 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, and policies were inspected. Children were observed participating in dramatic play, tabletop activities, playing outside, and circle time.

Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection.

Standard 22.1-289.035-B-4
Based on record review and staff interview, 1 of 5 staff files indicated that the center failed to obtain an out-of-state child abuse and neglect search request by the end of the 30th day of employment.

Evidence:
1. There was no documentation of an out-of-state central registry checks for Staff #4 hired on 09/06/2022.

2. Staff #4 indicated living in two other states other than Virginia within the last five years on their sworn disclosure statement dated 05/13/2022.
Plan of Correction: Request for out of state (MD and HI) have been submitted. Once documentation has been given, will submit to licensing.
Standard 8VAC20-780-140-A
Based on record review and staff interview, 1 of 5 children records indicated the center failed to ensure that all physical examination records were obtained before the childs attendance or within 30 days after attendance.

Evidence: There was no documentation of a physical examination for Child #2 whose first date of attendance is 9/18/2023.
Plan of Correction: Child?s parent had the VA State physical form in her possession. It is now in the student file. Physical was dated 9/28/2023

Based on record review and interview, 1 of 5 staff records is missing documentation that two or more references as to character and reputation as well as competency were checked before employment

Evidence: Staff #5 (DOH:10/01/2021) is missing 2 out of 2 references.
Plan of Correction: Recommendation letters have been received and were placed in the staff file.
