Inspector Notes

An unannounced monitoring inspection was conducted on March 26, 2024 from approximately 2:35 pm to 3:40 pm. Provider was present with four children. Children were observed napping, playing freely and eating an afternoon snack. Four child, two caregiver and one household member records were reviewed. Violation found is noted on the violation notice.

Based on review, provider did not ensure a caregiver has an updated background check
every 5 years.

Evidence: The record for caregiver #2 has a most recent Office of Background Check
fingerprinting letter dated March 8, 2018 which is more than 5 years old.
Plan of Correction: Background check was completed and results letter received 4/2/2024.
