Inspector Notes

An unannounced monitoring/renewal inspection was conducted today from 2:50-3:50. There were 8 children for a total of 24 points directly supervised by 3 staff and another arrived after inspection started. The home, 4 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed waking from nap, playing freely and eating snack. Diapering procedures were also observed. The Provider asked about a household member taking over license and instructions given about contacting Fairfax Office for training. The Provider also had paperwork indicating she had another address but the Provider stated she goes on weekends. There was an assortment of age appropriate materials for children. The entire home was walked through and Provider's dog was also present.. Areas of non-compliance are identified in the Violation Notice.

Based on record review, it was determined that caregiver #4's Central Registry check was not resubmitted before 5 years since last Clearance.
Evidence:
1. Caregiver #4's Central Registry was completed 2/23/2018.
2. Provider thought it had been resubmitted but was unable to locate.
Plan of Correction: We will make sure that a new one is sent.
