During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 02/24/2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 4 caregivers; the point total was 26. The inspector reviewed 4 children?s records and 3 caregiver/household members' records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Caregivers #2 and #3 did not have documentation of obtaining any training hours in 2024. Caregiver #3 stated they did not do any trainings in 2024.
The record for Child #2 was missing written authorization for emergency medical care or a written objection to provision of medical treatment on religious or other grounds. The child was present during the inspection and had been in care for a month.
Caregivers reported that diaper ointment and lotion for child #4 have been used, however no written parent authorization has been obtained. Child #4 was present.
