During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/13/2025, as a part of the licensure period. The on-site inspection began at 8:50am and ended at 10:05am. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 34. The inspector reviewed 5 children?s records and 4 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-A
A Sworn Statement is required for adults living in a family day home.

Household Member #2 did not have a sworn statement in the file. The provider stated they weren't aware this document was required for household members. Household member is over the age of 18.
Plan of Correction: Provider will have household member complete sworn statement today.
Standard 22.1-289.036-B-2
Adult household member are to submit to fingerprinting within 30 days of 18th birthday.

Household member #2's fingerprints were obtained 6 months after turning 18.
Plan of Correction: Provider will track due dates to have background clearances completed every five years for all adult household members and staff.

REPEAT VIOLATION
A child's record shall contain written authorization for emergency medical care, if parent cannot be reached.

Child #1 and child #3's records did not have written authorization for emergency medical care, if parent cannot be reached.
Plan of Correction: Provider will obtain written authorization to provide emergency medical care from parents today.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

Child 3#'s record of immunizations was obtained 2 weeks after child's first day of attending.
Plan of Correction: The provider will ensure immunization record is obtained prior to child attending daycare.
