A complaint was received in the licensing office on 06/21/2023 regarding forbidden actions and bottle feeding. On 06/29/2023, an on-site investigation was conducted. Interviews were conducted, records were reviewed and video footage was observed. At the time of this investigation, the preponderance of evidence did not support the allegation and the complaint is deemed not valid. Unrelated areas of non-compliance with applicable standards or law were found and documented on the violation notice issued to the program. Based on record review and staff interview, the record of each child on formula did not contain the brand of formula and the child?s feeding schedule.
Evidence: Staff #2 stated that they did not have documentation of the brand of formula or feeding schedule for children on formula.

Plan of Correction: A form was created that documents each infants feeding schedule and type of formula that is used. The forms are located in the infants file and a copy is provided to the teacher in the classroom.
