Inspector Notes

An unannounced Monitoring Inspection took place on November 13, 2023 between the hours of approximately 11:05 a.m. and 1:35 p.m. There were 10 classrooms observed with a total of 115 children with 17 staff within the supervision guidelines. The children were observed napping, eating lunch (vegetarian sloppy joes, cheesy zucchini, applesauce and bread), playing with blocks and magnets and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, and fire drill log was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. REPEAT VIOLATION

Based on record review, the center failed to obtain documentation that each child has received required immunizations.

Evidence: Child #5 (start date: 07/05/2022) did not have documentation of immunizations on file.

Based on record review, the center failed to ensure that all lead teachers met the program leader qualifications.

Evidence:
1. Staff #6 confirmed that Staff #5 has been an acting lead teacher for approximately 1-2 months. Staff #5 is not lead qualified.
2. Staff #6 confirmed that Staff #4 has been an acting lead teacher since June. Staff #4 is not lead qualified.

Based on record review and staff interview, the center failed to ensure that in each grouping of children at least one staff member who meets qualifications of a program leader was present.

Evidence:
1. Staff #4 works alone in the Toddler B- 2 classroom. Staff #4 is not lead qualified.
2. Staff #5 works alone in the Infant B classroom. Staff #5 is not lead qualified.

Based on observation and record review, the center failed to ensure that at least one staff in each classroom have a current certification in cardiopulmonary resuscitation (CPR) and current certification in first aid.

Evidence:
1. Staff #7 and Staff #8 were working together in the Intermediate A classroom. Staff #7 and Staff #8 do not have a current certification in CPR and First Aid.

3. Child #7 (start date: 08/28/2023) did not have a record at the time of inspection.
