Inspector Notes

This subsidy inspection was initiated by a move notice conducted by the inspector on 3/15/2023 beginning at 11:20am and ending at 12:10pm. There was 1 child in attendance at the time of the inspection. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor.

Based on review of the children's information, the Vendor did not have all required information on the children in care.

Evidence: The address and telephone number for each custodial parent's place of employment was missing for Child #1. The name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached was missing for Child #2.
Plan of Correction: The Vendor will talk with the parents and get the needed information from them.

Based on observation, the first aid kit did not contain tweezers.
