A subsidy complaint inspection was initiated on 01/11/2023 . A complaint was recieved by the department regarding allegations in the areas of behavorial guidance and training. The evidence gathered during the investigation supported the complaint of non-compliance with vendor requirements. Violations were issued.

Standard 8VAC20-790-550-5
Based on record review and staff interview, the vendor did not ensure documentation was completed for training, including the training, the date ,the total hours of the session, and the names of the trainer and/or of any sponsoring organization.

Evidence: Records did not have documentation of the orientation. Staff confirmed there was no documentation of orientation training with all required components in the 4 staff records reviewed.
Plan of Correction: Created a checklist for all new employees for training . And the center also having present staff complete the training checklist.
Standard 8VAC20-790-700-A
Based staff interiew and video footage review, the vendor did not ensure behavioral guidance shall be constructive in nature and age and stage appropriate and shall be intended to redirect children to appropriate behavior and resolve conflicts.

1- Video footage reviewed and staff members confirmed, an incident in November 2022 in the infant room, video showed Child #1 was moving around on the cot and walking around the room. Staff #1 came over and sat down holding an infant. Child #1 was laying on the cot. Staff #1 was sitting and put her leg over the back of the calves of child #1 for approximately 2 minutes.

- Staff #2 stated that Child #2 was put in the high chair one time to calm down because of the child?s behaviors.
Plan of Correction: The center has coached the staff not to hold a child down to fall asleep. If more assistance is needed staff will walkie for help in the classroom.

We removed highchairs from classrooms. We are using redirection for all children in the classrooms that are making bad chioces.
