Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 03/19/2021 and concluded on 03/24/2021. The director was contacted by email to initiate the inspection. There were 17 children present and 6 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-185-140-A
Based upon review of two children's records and staff interview, the facility has not ensured that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attending.
Evidence:
1. The record provided for child 1 indicates an enrollment date of 1/10/2021. Documentation of a physical examination was not in the record.
2. Staff 1 verified that documentation of a physical examination was not obtained for child 1.
Plan of Correction: The facility responded with the following:
The child's most recent doctor visit was in February 2021. The parent will be notified to provide documentation of a physical examination. All children's records will be checked to ensure the required documentation is obtained and filed.
