During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 02/12/2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to Administration, Staffing and Supervision and Programs..The on-site inspection began at 12:45 PM and ended at 2:40 PM. The inspector reviewed compliance in the areas listed above. CDC: There were 16 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records.
This inspection included:

? observations.
Information gathered during the inspection determined a non-compliance with applicable standards or law, and a violation is documented on the violation notice issued to the program.
Violations
1Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the names, addresses and telephone numbers of each parent and the names, addresses and telephone numbers of two persons to be contact in an emergency when a parent cannot be reached.
The record provided for child 3 did not contain the name, address and telephone number of one parent. The record provided for child 4 contained only one emergency contact person.
All children's records will be check to ensure they contain all required information using the child file checklist.
