When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 11/24/2025, staff did not ensure that care and protection of children when child 1 sustained an injury to the lip. Staff 1 and 3 confirmed the child's injury however were unaware how the injury occurred and failed to follow up with investigation and/or reporting.

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group. In a combined grouping in which an infant is in care, the ratio is no more than 4 children per caregiver.
Staff confirmed that on 11/24/2025, there was a combined grouping of 6 or 7 children ages 13 months through two years in a group with only staff #2 while staff #1 went into another classroom for approximately ten minutes. Staff #3 was sitting in a doorway between classrooms working in a notebook.

Staffing ratios were reviewed by KKCC Administrator on 12/09/2025.
Administrator will continue to monitor ratios and utilize ratio checklist daily to ensure staff are following ratio protocols.

An all staff training was completed by the KKCC Administrator on 12/09/2025, reviewing procedures for documenting, treating, and reporting injuries. The training also included a refresher on how to use the ProCare App to report minor injuries.
