Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
The record provided for staff 1 did not have documentation that staff 1 received the required orientation training no later than seven days of the date of assuming job responsibilities. Staff 1 was on duty with the children during the inspection.

Four children between the ages of 16 months and three years were observed in the back play room playing with 2 diecast metal cars designed for use by children 3 years and older.
There were a long thin carpet and a rectangular carpet in the toddler classroom with the corners curling. The small carpet placed under the gate between the classrooms has curling edges.
There was a long dangling cord to an intercom unit placed on a windowsill in the back play room.

Children ages sixteen months through two years were seated at the tables for twenty minutes while waiting for lunch to be served. The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. The changing table shall be only used only for diapering or cleaning children.
Staff 1 changed a toddler's diaper in the rest room where there were no cleaning or sanitizing agents to clean/sanitize the changing table after the diaper was changed.
