Inspector Notes
An unannounced Monitoring Inspection took place on 04/11/2023 between the hours of approximately 9:00 a.m. and 11:10 a.m. There were 6 classrooms observed with a total of 78 children with 15 staff within the supervision guidelines. The children were observed participating in circle time, playing in organized centers, napping, and preparing to go to the playground. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and injury prevention plan was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 10Evidence:
1. Staff #1 (start date: 09/26/2022) had a Sworn Statement dated 09/27/2022.
2. Staff #4 (start date: 03/13/2023) had a Sworn Statement dated 03/29/2023.
Evidence: At the time of inspection, Child #1 (start date: 07/18/2022) did not have documentation of immunizations on file.
Evidence: Child #4?s (age: 21 months) most recent documentation of immunizations was dated 09/15/2022.
Evidence:
1. Staff #1 (start date: 09/26/2022) had a tuberculosis screening that was dated 11/09/2022.
2. Staff #4 (start date: 03/13/2023) had a tuberculosis screening that was dated 03/21/2023.
Evidence:
1. There was a bottle of hair mousse (warning label says to keep out of reach of children) in a yellow bin on a shelf, inside the Preschool classroom?s bathroom, accessible to children.
2. In the Pre-K 1 classroom, there were five cans of shaving cream (warning label says to keep out of reach of children), in a bin on a wire rack by the sink, accessible to children.
Evidence:
1. At the time of inspection, Child #6 and Child #7 were napping in cribs that were located next to each other.
2. There was approximately seven inches between the sides of Child #6 and Child #7?s cribs.
Evidence: In the Preschool (two-year old) classroom?s bathroom, there were six containers of sunscreen located on a shelf in a blue bin, accessible to children.
Evidence: There was one bottle of insect repellant located in a blue bin on a shelf, inside the Preschool classroom?s bathroom, accessible to children.
Evidence: At the time of inspection, Child #1?s (start date: 07/18/2022) record did not contain documentation of viewing proof of the child?s identity and age.
Evidence:
1. At the time of inspection, Staff #1 (start date: 09/26/2022) did not have documentation that two or more references were checked.
2. At the time of inspection, Staff #2 (start date: 02/13/2023) only had documentation that one reference was checked.
3. At the time of inspection, Staff #4 (start date: 03/13/2023) did not have documentation that two or more references were checked.
