A Subsidy Health and Safety inspection was conducted on April 19, 2024, to review supplemental health and safety requirements. During a self reported investigation that was received by the licensing office on March 19, 2024, an investigation was initiated in regards to a violation of the vendor agreement. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Based on interview, the vendor did not ensure compliance with the terms of the vendor agreement.
Evidence:
1) The Vendor Agreement states, ?The Vendor shall not be in possession of or swipe any parents? or guardians? VA-ECC card.?
2) Witness #1 confirmed that the center has possession of parents VA-ECC cards.
3) Witness #1 stated that the VA-ECC cards are kept in a room that the parents do not have access to.
4) Staff #1 stated that Staff #2 has been in possession of VA-ECC cards.
5) Staff #1 stated that they have seen Staff #2 help parents swipe the VA-ECC cards.
6) Staff #2 confirmed that the center has had possession of 2 parents VA-ECC cards.
7) Staff #2 confirmed that the center no longer has possession of any parents' VA-ECC cards.
