Violations

2
Standard 8VAC20-800-60-B
Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence;
Child record B does not include the information for the parent's medical insurance policy name and number.
Plan of Correction: I will ask the parent for the information at pick up today.
Standard 8VAC20-800-900-A
Based on interviews, it was determined that the provider did not assist in preventing choking by preparing one possible choking hazard food in a manner to reduce the risk of choking.
Evidence:
The children were served grapes with lunch today. The provider did not slice the grapes into small pieces.
Plan of Correction: I cut the grapes but didn't slice them. If I serve grapes again, I will make sure they are sliced properly.
