Inspector Notes
An unannounced renewal inspection was conducted today between 11:40am and 12:50pm. There were 6 children present with the Provider for a total of 12 points within the supervision guidelines. The children were observed watching an education video about the letter M, playing freely with legos and magnet tiles, cleaning up and eating lunch (egg soup over rice, corn, pasta, and ham and sausage). Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
6Evidence:
1. Caregiver #1's most recent fingerprint results are dated 5/15/208.
2. Household member #1's most recent fingerprint results are dated 5/15/2018.
3. The provider confirmed that repeat fingerprinting has not been completed when the background checks expired in 5/15/2023.
Evidence: Child #6 (date of enrollment: 96/2022) record does not contain a physical exam.
Evidence: Household member #2's most recent TB screening is dated 7/2/2021. The provider stated that they completed the TB screening but did not give the results to the provider.
Based on record review, children's records did not contain all required information.
Evidence: Child #6's record did not contain the families medical insurance policy number.
Evidence: The children were observed cleaning up from playing and going straight to the table to eat lunch. The provider squirt hand sanitizer in each child's hand prior to the children beginning to eat.
Evidence: The children were not served anything to drink during lunch.
