Inspector Notes

An unannounced inspection was conducted on 07/21/2023 from approximately 2:25 pm to 2:45 pm in response to a complaint received on 7/20/2023 regarding allegations in the area hazardous substances and other harmful agents.

Observation of the center director?s office and interview with the center director was conducted. The preponderance of the evidence gathered during the investigation supports noncompliance in the area of hazardous substances and other harmful agents, the complaint is determined to be valid.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on observation, the center failed to ensure hazardous substances such as
cleaning material, insecticides, and pesticides were kept in a locked place using a safe
locking method that prevents access by children.

Evidence:
1. A bottle of home defense insect killer was observed being used to prop the center director?s office door open.
2. During the inspection a child was observed in the center director?s office.
Plan of Correction: The bottle of home defense was locked up immediately during the inspection and a door stopper is being used to hold the door open.
