Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. The documentation for the screening shall be submitted at the time of employment and prior to coming into contact with children. The record for Staff #4 did not contain documentation of a negative TB screening. Staff #4 has been employed for 10 days, supervising children.
Plan of Correction: The staff member was informed a tuberculosis screening must be obtained before the end of the week.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
Classroom #1 had a floor mat that contained multiple tears in the equipment.
Classroom #2 had an area of peeling paint underneath the window. The area was accessible to the children.
Plan of Correction: We will have someone repair the wall and the floor mats were removed during the inspection.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children. The unlocked closet located in Classroom #3 contained three cleaning chemicals labeled ?Keep Out of the Reach of Children?.
Plan of Correction: Corrected during the inspection and we will continue monitor the classrooms to ensure all hazardous substances are kept locked in a designated area.

Prescription and nonprescription medication shall be given to a child, only with written authorization from the parent. Medication was present at the center for Child #7. The medication was given to Child #7 daily and documented on the medication log. There was no written authorization on file signed by the parents.
Plan of Correction: The medication will no longer be administrated until the required authorizations are obtained.
Standard 8VAC20-780-560-G
When food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Eight water cups located in Classroom #4 were not labeled or dated. The eight water cups were provided from the children?s homes.
Plan of Correction: Parents and staff will be reminded all items from home need to labeled and dated daily.
