Inspector Notes

A remote inspection was conducted based on the center?s report of an incident that occurred on November 16, 2023. Documentation and video footage were reviewed. The preponderance of the evidence gathered during the investigation support non-compliance with standards. A violation notice was issued. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Based on review of classroom video provided by the center, staff interview, and documentation, it was determined that staff used the forbidden action of rough handling of children.

Evidence:
On November 16, 2023, S1 roughly handled child, C1 by grabbing C1 from a bike on the playground by her left arm causing an injury. The child was taken to the hospital for treatment.
Plan of Correction: Immediate termination of the employee and retraining for all other employees to be completed by 12/1/23.
