During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 10/29/2024 and completed on 10/29/2024. The on-site inspection began at 10:50am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 caregiver; the point total was 13. The inspector reviewed 6 children?s records and 1 caregiver record on-site. This inspection included document review, tour of the home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/13/2024. A POC submitted after this date will not appear on the public website.
Violations
6There were no physical exam records available for child #1 (enrolled for over 3 months) and child #2 (enrolled for 7 months).
An updated TB screening for Caregiver #1 was completed 4 months after being due.
Caregiver #1 obtained 1.5 of the 16 required hours in 2023.
The provider shall keep a written record of children in attendance each day.
There was no record of attendance for child #4, who was in care on the date of the inspection.
The record for child #2, enrolled for seven months, was missing the following required components: the name, address, and phone number of 1 designated person to contact in case of emergency. The record for child #3, enrolled for over 1 year, was missing the following required components: phone number and address for 1 parent, employment information for 1 parent, and work phone number for both parents.
A prescription medication for child #6 was observed in the child?s backpack, not in a locked container, making accessible to children.
