Inspector Notes

An investigation was initiated on 5/12/22 in response to a complaint received by the department on that same day regarding concerns in the areas of supervision and toileting and diapering procedures. A second complaint was received on 5/13/22 regarding the center?s toileting and diapering procedures. An on-site inspection was completed by licensing inspectors on 5/31/22 from 9:40am until 1:05pm. Interviews with staff were conducted, a tour of the entire building and outdoor play area was completed and video footage and center documents related to the complaint were reviewed. Based on the information gathered during the investigation, the complaint regarding the center?s toileting and diapering procedures is determined to be valid. In regards to the complaint about supervision, there is insufficient information to determine the complaint is valid. Violations are cited on the violation notice and were discussed with center management at the conclusion of the investigation.

Based on staff interviews, the center did not ensure that a nonabsorbent surface for diapering or changing shall be used and that for children younger than three years, this surface shall be a changing table or countertop designated for changing.

Evidence:
1. Staff 1 and staff 2 stated it is usual practice to change the diapers and pull-ups of children under the age of 3 years while they are standing up in the bathroom instead of on the changing table.
2. Staff 3 (center owner) and staff 4 (center director) confirmed that it is the usual practice of the center to change children under the age of 3 years while they are standing up in the bathroom instead of on the changing table in the bathroom.
Plan of Correction: The center responded with the following: Going forward, staff will change all children under the age of 3 years on the changing table. We will have another staff close by in case assistance is needed due to any special needs of the children.
