Inspector Notes
An unannounced inspection was initiated on May 6, 2024, and concluded on June 6, 2024, in response to a complaint received by the licensing office on May 6, 2024, relating to allegations of staffing and supervision and programs. The inspector reviewed four children?s records and one staff record on-site on May 8, 2024. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/12/2024. A POC submitted after this date will not appear on the public website.
Violations
21. Child #1 (age 3) and Child #2 (age 4), were wrestling directly in front of Staff #1 on April 23, 2024. A third child in the classroom attempted to get Staff #1?s attention on two separate occasions pointing at the two children wrestling. Staff #1did not respond, and continued to sit and scroll on their personal cell phone.
2. The two children continued to wrestle in another area of the classroom and Staff #1 did not redirect them but continued to stay on their personal cell phone. The children were wrestling for at least 10 minutes. While wrestling, one child received several bite marks and bruises on their back.
3. Staff #1 confirmed that children were wrestling and stated and confirmed that they did not physically intervene to redirect them.
4. Staff #1 confirmed that personal cell phone usage is prohibited, per the center?s policy, and admitted doing ?personal stuff? on their phone while in the role of supervising children.
1. On April 23, 2024, Staff #1 lifted Child #1 (age 3) off the ground by the arm to have the child stand up. Staff #1 walked the child across the room holding their arm then pushed Child #1 to another side of the classroom.
2. Staff #1 confirmed grabbing Child #1 to move them to another area of the classroom.
3. On April 23, 2024, Child #3 (age 3) was restrained in a highchair and fell to the floor with the highchair still attached to their body. The child was able to prop themselves upright in the highchair and fell two additional times within a 5 minute timeframe. After the third fall to the floor, Child #3 crawled across the room with the highchair attached to their body.
4. Staff #1 stated they had no knowledge of Child #3 falling three separate times while restrained in the highchair and confirmed that they were responsible for supervising and caring for Child #3.
High chairs have been removed permanently.
