During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 7/24/24 and completed on 7/24/24 with the local agency in response to a self-report received by the licensing office on 7/8/24 relating to supervision of children. The Department issued a Special Order to prohibit new admissions of preschool age children for a period of thirty (30) days and to require the licensee to contact parents, guardians, or other responsible persons (families) regarding health and safety violations on 8/27/24 as a result of high-risk violations. The inspector reviewed 1 children?s record on-site on 7/24/24. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/16/24. A POC submitted after this date will not appear on the public website.

Children 10 years of age and older shall be within actual sight and sound supervision of staff.

On July 8, 2024, child 1, who is a 13-year-old with special needs, was left out of sight and sound supervision of staff for approximately eight minutes. Staff 1 stated that she left child 1 in the classroom to use the restroom while she took the other children to the playground. Child 1 then exited the building and was unable to get back inside. The child was found alone outside by a person not associated with the facility. Staff 1 stated she found child 1 in the parking lot of the facility with staff from a neighboring business approximately 8 minutes after first leaving her in the classroom.
Plan of Correction: The center responded with the following: Staff will ensure that children 10 years and up will remain within actual sight and sound at all times.
