Inspector Notes
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
This inspection was initiated and concluded on 09/24/2021. The Provider was contacted and a virtual inspection was conducted. There were 4 children present (9 points), ranging from 20 months to 3 years in age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition (meals and snacks brought from home). A total of 4 children records, 1 caregiver record, and 2 household member records were reviewed.
Information gathered during the inspection determined that there were non-compliances with applicable standards, code, and/or law. These violations were documented on the Violation Notice issued to the program.

1. On the date of inspection (09/24/2021), TB documentation for Household Member #2 was not available for review.
2. The Provider stated (during an interview conducted on the date of inspection) that Household Member #2 is away and will not return until next week.
3. This requirement for Household Member #2 came into effect in mid August 2021.

1. On the date of inspection (09/24/2021), TB documentation for Household Member #1 was dated - 04/12/2019.
2. This finding was confirmed by the Provider on the date of inspection.

1. On the date of inspection (09/24/2021), a SDS was not available for Household Member #2.
2. It was also determined that this documentation was not requested within 30 days prior (in mid July) of Household Member #2 becoming 18 years of age (in mid August).
